Code 99495 ( Avg. Reimbursement $163 ) : moderately complex MDM and face-to-face visit within 14 days of discharge
Code 99496 ( Avg. Reimbursement $230 ) : highly complex MDM and face-to-face visit within 7 days of discharge
*Both codes also require direct communication with the patient or caregiver within 2 days of discharge ( via telephone, direct contact or electronic means )
