Social workers support patients and families by sharing their understanding of the psychological functioning of the person and then involving the support and resources of the community.

This may include psychotherapy, supportive counseling, grief counseling, or helping a person strengthen their network of social services.  Social workers work with a RN Case Manager, the patient’s physician, and therapists.

Social workers often help clients develop their independence – linking them with services like delivered meals, transportation.  They may also help assess a patient’s needs for more care when a transition appears necessary (potentially to independent living or to assisted living or a nursing home).
