Our physical therapy team responds to the RN case manager’s request for services. A certified Physical Therapist meets with the client to assess their needs, often reviewing notes from previous therapists (if they’ve come from a rehabilitation stay in a hospital or nursing facility). Depending on what is needed, a plan is written, and a schedule set.
Therapy may include:
- Strengthening
- Range of Motion
- Pain Relief
- Therapeutic exercise
- Balance training: endurance, flexibility, safety education
The certified Physical Therapist also assesses the challenges of the client’s home and will recommend changes to make it a safer environment (maybe the use of grab bars for the shower, removal of throw rugs, better lighting for the hallways, a raised seat and arms for the toilet…).
Balance Therapy reduces the risk of falls and is key to helping people maintain a healthier life.
